Program Information and Forms

Our Concierge Functional Medicine Program is designed to provide personalized, ongoing support through a comprehensive partnership focused on optimizing health, longevity, and overall wellness. Membership is available for individuals, couples, and families and includes continued access to your care team, individualized treatment planning, and ongoing health optimization services.

Program fees are $90 per month for an individual adult, $150 per month for a couple, and $30 per month for each child. To foster continuity of care and allow adequate time for meaningful health improvements, enrollment is structured in six-month membership terms. Membership fees are collected in advance for each six-month period and will automatically renew for subsequent six-month terms unless a cancellation request is submitted prior to the renewal date.
