Welcome to Springfield, MO and the start of your adventure with Prime, Inc. One of your most important steps during your first week here is your DOT physical. If you do not have this done you will not be able to drive.

 

Before you come to the clinic for your physical you will need to fill out your paperwork.  Below are the steps you need to complete to get this done​ once you click the button below.

• Enter the date you are scheduled to start orientation.

• Enter your personal information-name, date of birth, age, address, etc.

• Next you will be asked a series of questions about your health history. Answer the questions to the best of your ability.  Don’t worry if you make a mistake, we will fix it when you come to the clinic.

• If you have a history of work related injuries or military based disabilities you will enter that information next.

• The final set of questions asks about your physical abilities.

• Next you will be asked to enter the provider you are seeing.

  • You will need to enter the name of the provider from the piece of paper you received when you checked in to the hotel.
