Covid-19 Vaccine

The past year has been incredibly challenging for everyone on a global level.  There has been much tragedy and loss of life.  We are very grateful to the scientific community in all their efforts to develop a vaccine for Covid-19.  We finally have a light at the end of the tunnel and look forward to getting closer to the normalcy we once had in our lives.

This webpage is designed to assist you in scheduling for your vaccine with Trinity Healthcare. Once you receive the injection we will want to observe you for 15 minutes. We do try to keep things as streamlined as possible however, please allow for 30 minutes for the whole process.

*Important update-Starting August 10 we will now be offering the Pfizer vaccine to patients between the ages of 12 and 18 or those that have received their first dose of Pfizer elsewhere. Please make sure you fill out the appropriate consent form below and only select the Pfizer paperwork if you meet the above criteria. Filling out the incorrect form may result in not receiving a vaccine.

Please follow the steps below in order to receive your covid vaccine.

​

1. Schedule your first and second shot with this link. You will have to schedule the first shot and then repeat the process for the second shot 4 weeks later. We recommend you do this immediately so appointment slots do not get filled up. Failing to schedule your second appointment may mean not having a dose for you.

 

2. Fill out the "Covid-19 Vaccine Screening and Consent Form". This will come to us electronically. You MUST fill this out before each shot.

Attention Drivers:

With the large response we have received for people wanting vaccines we have created a sign-up link specifically for drivers that offers more flexibility to accommodate the sometimes unpredictable schedules.  You will still need to sign up for an appointment time using the "Driver vaccination" button above, however you may come in anywhere from 9 to 12 and 1 to 4. Please make sure you fill out the “Screening and Consent Form” so we have your paperwork ready for you. This will help expedite your visit and get you on your way as quick as possible. If you find you have to cancel your appointment, please let us know.

You will not be charged for receiving the vaccine, however if you have insurance please bring your insurance card with you to your visit. Due to government regulations we must have this on file
